Follow this step-by-step guide to complete the Memento Mori achievement:
Step 1: Play on the Thebes Map
The first step is to play on the Thebes map. To access it, queue for a quick or competitive match. Make sure you play a public match, as completing this in a custom match will not grant you the achievement.
Step 2: Locate Apocalypse's Sarcophagus
The next thing you need to do is find Apocalypse's Sarcophagus. It is located in front of the defenders' front spawn room in the final area of the map.
If you're playing as a defender, you'll need to wait until the attacking team captures the first checkpoint. Once that happens, you'll respawn in the final area's front spawn room. Exit the room and look at the coffin positioned between the two spawn doors, that's Apocalypse's Sarcophagus.
If you're playing as an attacker, you'll need to reach at least the second checkpoint. After capturing it and the doors open to the outdoor area, immediately turn right as you exit. You'll see a room in the corner, just behind a set of stairs with two cat statues on the side. Inside, you'll find Apocalypse's Sarcophagus.
